NEW PATIENTS: To make a telemedicine appointment, please Request an Appointment online, and then fill out the Online Registration Form.

ESTABLISHED PATIENTS: Please just Request an Appointment online as usual. However, if there are any changes to your address, insurance, health issues, etc., please fill out the Online Registration Form as well.

Once your request for a telemedicine visit is received, you will be sent a link to the appointment meeting. We use Doxy.me for our telemedicine appointments, a secure and HIPPA compliant medical platform - no need to download any apps or software.
